Immunizations

Please remember to have your child's immunizations updated and a current copy needs to be on file at the school per Georgia law. We are required to have form 3231 this can be obtained from either your physician or Chatham County Health Department.

EED (Ear, Eye and Dental)
Form 3300 is now required by law for any student entering a Georgia school for the first time either as a four year old or a new student in any grade pre-K through 12th grade. These forms are a screening tool and can be completed by either your health care provider or the Chatham County Health Department.

RE:  HEAD LICE 

It is important for you to check your child and other family members’ heads each week during the next few months for head lice.                                                                                           

 TO CHECK FOR HEAD LICE

  1. Use a bright light.
  2.  Have the individual lean his head forward, then systemically spread the hair apart. 
  3. Look for live lice or nits at the back of the neck, around the ears and on the crown.
  4.  Should you find head lice, treat the individual immediately.  

GUIDELINES FOR THE MANAGEMENT AND TREATMENT OF HEAD LICE

  1. Obtain a lice treatment shampoo (pediculicide) from your pharmacist or physician.
  2.  Use the shampoo as directed on the label.  This includes massaging the shampoo on the entire surface of the hair and scalp, then removing all nits with a fine tooth comb. 
  3.  Wash clothes, bed linens, towels, hats, coats, and other washable items in hot soapy water, then dry in a hot dryer for at least 20 minutes.  Laundering should occur within 24 hours of shampooing hair. 
  4. Vacuum all mattresses, pillows, blankets, rugs, upholstered furniture and fabric car seats. Discard the vacuum cleaner bag in an outside receptacle.  
  5. Dry clean or seal in plastic bags for 10 days any articles that cannot be washed or dried, such as stuffed animals or pillows.
  6. Clean combs, hairbrushes, and barrettes with lice treatment shampoo, medicated creme rinse or boiling hot water. 
  7.  Retreat 7 to 10 days after the initial shampoo treatment to kill any newly hatched louse.
  8. Notify the school, and your children’s playmates, if lice or eggs are found in your child’s hair.
